HCTM and the National Archives of Malaysia Sign Agreement on Medical Records Disposal Schedule
Hospital Canselor Tuanku Muhriz (HCTM), Universiti Kebangsaan Malaysia (UKM), through the Health Information Department, held the Signing Ceremony for the Mutual Agreement on the Medical Records Disposal Schedule between HCTM and the National Archives of Malaysia at Sukmaria, Clinical Block, HCTM, on 12 March 2026.
HCTM Director, YBhg. Dato’ Dr. Azmi Baharudin, described the ceremony as a significant milestone, marking HCTM’s official adoption of the Medical Records Disposal Schedule endorsed by the National Archives of Malaysia. He stated that this recognition further strengthens the governance and management of medical records within the institution.
“Medical records are essential documents that capture patients’ treatment histories and serve as key references in ensuring continuity of care.
“Beyond that, these records are also crucial in fulfilling legal requirements, supporting research activities, and facilitating health statistics reporting,” he said in his speech during the ceremony.
He further emphasised that the management of the medical records lifecycle, from creation to disposal, must be carried out systematically while adhering to all applicable acts, policies, regulations, and procedures. This initiative is vital in ensuring that the security and confidentiality of patient information remain protected in accordance with the requirements of relevant stakeholders.
The ceremony was also attended by YBrs. Tuan Haji Zaidin Mohd Noor, Director General of the National Archives of Malaysia, directors from various divisions of the National Archives of Malaysia, HCTM Deputy Directors, Puan Hanuzah Omar, Head of the HCTM Health Information Department, as well as senior officers from both HCTM and the National Archives of Malaysia. The collaboration reflects HCTM’s ongoing commitment to strengthening healthcare governance through effective, secure, and accountable medical records management practices.
