Opening Ceremony of Patient Safety Week
The Opening Ceremony of Patient Safety Week at Hospital Canselor Tuanku Muhriz (HCTM), Universiti Kebangsaan Malaysia (UKM), was held on 18 September 2025 (Thursday), at the Gallery of the Chancellor Tuanku Ja’afar Medical Education Complex, HCTM. The event was officiated by YBhg. Dato’ Dr. Azmi Baharudin, Director of HCTM.
This year’s Patient Safety Week carries the theme “Safe Care for Every Newborn and Every Child”, highlighting the critical importance of active roles in ensuring the safety and quality of care for children. HCTM remains steadfast in its commitment to patient safety and quality by enhancing incident reporting systems, conducting regular clinical audits, and implementing relevant training and simulation exercises to ensure optimal preparedness.
This event is not merely an annual program but a strategic platform to renew commitments in improving systems, strengthening procedures, and enhancing communication between healthcare teams and patients to achieve excellence in healthcare.
Also in attendance were YBrs. Puan Wan Omi Wan Yussop, Head of the Finance Department; YBrs. Puan Masrifah Zakaria, Head of the Nursing Services Department; YBrs. Dr. Sharifah Azura Salleh, Head of the Infection Control Unit and Chairperson of the Organising Committee, Heads of Departments and members of the HCTM staff.
HCTM Successfully Won 6 Awards at the 10th UKM Bitara Awards Ceremony 2025
A captured moment from the 10th UKM Bitara Awards 2025 Ceremony, held on 9 September 2025 (Tuesday), at the Tun Abdul Razak Chancellor Hall (DECTAR), Universiti Kebangsaan Malaysia (UKM). This event serves as a recognition of excellence by the university towards its responsibility centres and UKM community members in the areas of academics, research, internationalisation, income generation, and human capital development.
Hospital Canselor Tuanku Muhriz (HCTM), UKM proudly secured six (6) award categories, the Bitara Premier Award in the Service Centre Category, Bitara Mass Media Award (Non-Academic Category), awarded to Mr. Shahrol Iza Suid Albana, Department of Multimedia & Broadcasting, Bitara Industry & Community Engagement Award (Non-Academic Industry Category), awarded to Ms. Rohana Mukahar, Department of Medical Rehabilitation Services, Bitara Initiative Award (Management & Professional Category), awarded to Dr. Afliza Abu Bakar, Department of Emergency Medicine, Bitara Initiative Award (Support Staff Category), awarded to Ms. Siti Hajar Mat Desa, Department of Nursing Services and Bitara Funding Award (Group Category), awarded to the Department of Radiology. Winners received certificates, trophies, and mock cheques ranging in value from RM2,000 to RM5,000.

Service Awards Ceremony (MAP) 2025
The Hospital Canselor Tuanku Muhriz (HCTM) Service Awards Ceremony (MAP) of Universiti Kebangsaan Malaysia (UKM) for the year 2025, themed “Southeast Asian Tradition,” was held with grandeur on Friday, 22 August 2025, at 8:30 PM at the Bangi Avenue Convention Centre (BACC). The ceremony honored 444 award recipients, which included the 20-Year and 30-Year Service Awards, as well as the 2025 Excellent Service Awards.
This recognition aimed to appreciate the tireless efforts, dedication and high commitment of HCTM staff in carrying out their duties and responsibilities. The welcoming speech was delivered by YBhg. Dato’ Dr. Azmi Baharudin, Director of HCTM, followed by a speech from YBhg. Prof. Emeritus Dato’ Dr. Mohamad Abd. Razak, Chairman of the UKM Board of Directors. The awards were presented to 304 recipients of the 2025 Excellent Service Award (APC), 139 recipients of the 20-Year Service Award, and one (1) recipient of the 30-Year Service Award.
The event also featured the launch of two (2) new digital applications, myHCTM and ePesakit@HCTM. The launch, officiated by YBhg. Prof. Emeritus Dato’ Dr. Mohamad Abd. Razak, aimed to enhance the efficiency, user-friendliness and integration of the healthcare service delivery system. The myHCTM app consists of modules for HCTM Staff and HCTM Management, while ePesakit@HCTM offers patients and their families direct access to services such as pre-registration, information updates, and clinic appointments.
The evening concluded with a heartfelt tribute and token of appreciation to former HCTM Director, YBhg. Professor Dato’ Dr. Razman Jarmin, and former Deputy Director (Diagnostic, Pharmacy and Support Services), YBrs. Associate Professor Dr. Hanita Othman, for their dedicated service and leadership.
Also in attendance were YBhg. Prof. Dr. Jamia Azdina Jamal, Acting Deputy Vice-Chancellor of Research and Innovation Affairs, YBhg. Prof. Ts. Dr. Faizal Mohamed, Acting Deputy Vice-Chancellor of Industry and Community Networking Affairs, YBrs. Tuan Haji Zanudin Mohd Daud, UKM Registrar, YBrs. En. Hazmir Hj. Zainal, Chief Librarian of UKM, YBrs. Encik Mursidi Ahamad, Deputy Director (Administration) cum Chairman of the Ceremony Committee, members of the HCTM Management, Department Heads, and celebrated HCTM staff.
