Case Summary

“When the jerks won’t stop…”

Madam W, A 42-year-old lady presented with altered behavior for 4 days. At the ED, she was noticed to have jerky movement of of right lower limb for 2 minutes. She was unresponsive during the episode. She had another 2 episodes which lasted 30 mintues in total. On examination, GCS E4V4M6, BP 152/90 mmHg, PR: 112/min, T-39.2C RR 20/min. No focal neurological deficit.

Hb 14.1 g/dL, WCC 11.1, Platelet 190,000; urea 8.2 mmol/L, creatinine 64 μmol/L, Na 146 mmol/L, K 4.1 mmol/L.
