{"id":70778,"date":"2021-02-25T01:20:05","date_gmt":"2021-02-25T01:20:05","guid":{"rendered":"https:\/\/hctm.ukm.my\/ent\/?page_id=70778"},"modified":"2023-11-14T11:00:34","modified_gmt":"2023-11-14T11:00:34","slug":"pengenelan-msqh","status":"publish","type":"page","link":"https:\/\/hctm.ukm.my\/ent\/pengenelan-msqh\/","title":{"rendered":"Pengenalan MSQH"},"content":{"rendered":"\r\n<h1 class=\"wp-block-heading\"><strong>APAKAH AKREDITASI HOSPITAL<\/strong><\/h1>\r\n\r\n\r\n\r\n<p>Akreditasi adalah alat atau kaedah yang digunakan bagi memantau dan mengukuhkan perlaksanaan \u2018good governance\u2019 dan \u2018good clinical governance\u2019 dalam memberikan perkhidmatan kesihatan kepada pesakit.<\/p>\r\n\r\n\r\n\r\n<p>Ia merupakan satu proses ke arah meningkatkan perkhidmatan kesihatan yang optima dengan menggunakan sumber yang sediada.<\/p>\r\n\r\n\r\n\r\n<p>Ia memberi penekanan dan fokus kepada Penambahbaikan berterusan, mempromosikan serta menyebarkan maklumat berkaitan Keselamatan Pesakit dan Budaya Kualiti bagi sesebuah jabatan atau organisasi.<\/p>\r\n\r\n\r\n\r\n<h1 class=\"wp-block-heading\"><strong>SIAPA YANG MEMBERI STATUS AKREDITASI<\/strong><\/h1>\r\n\r\n\r\n\r\n<p>MALAYSIAN SOCIETY FOR QUALITY IN HEALTH ( MSQH &#8211; OKT 1997 ).<br>Telah dilancarkan oleh Ketua Setiausaha Kerajaan pada bulan Mac 1998.<br>MSQH ( KKM-MMA-APHM )<\/p>\r\n\r\n\r\n\r\n<h1 class=\"wp-block-heading\"><strong>FAEDAH<\/strong><\/h1>\r\n\r\n\r\n\r\n<p>Kepuasan Pelanggan ( K )<br>Kepuasan Kakitangan \/ Anggotakerja (K)<br>Perkhidmatan Berkualiti ( P )<\/p>\r\n\r\n\r\n\r\n<p><strong><em>SECARA RINGKAS, DIFORMULAKAN SEBAGAI (KKP)<\/em><\/strong><\/p>\r\n\r\n\r\n\r\n<h1 class=\"wp-block-heading\"><strong>FOKUS \/ PERKARA UTAMA DALAM PROSES AKREDITASI<\/strong><\/h1>\r\n\r\n\r\n\r\n<ol class=\"wp-block-list\"><li>Organization and Management<\/li><li>Human Resource Development And Management<\/li><li>Policies And Procedures<\/li><li>Facilities And Equipment<\/li><li>Safety and Performance Improvement Activities<\/li><li>Special Requirements<\/li><\/ol>\r\n\r\n\r\n\r\n<h1 class=\"wp-block-heading\"><strong>CARA PENILAIAN AKREDITASI EDISI 5<\/strong><\/h1>\r\n\r\n\r\n\r\n<p>Rating 1 @ 2 @ 3 @ 4<br>Rating 1 = Poor\/Rendah<\/p>\r\n\r\n\r\n\r\n<p>( 0 \u2013 39 % ) Rating 2 = Fair\/Sederhana<br>( 40 \u2013 59 %)&nbsp;Rating 3 = Good \/ Baik<br>( 60 \u2013 79 % )&nbsp;Rating 4 = Excellence \/ Cemerlang<br>( 80 \u2013 100 % )&nbsp;Rating LULUS adalah 3 dan 4<br>Rating GAGAL adalah 1 &amp; 2<\/p>\r\n\r\n\r\n\r\n<p><strong>Wajib dapat sekurang-kurangnya Rating 3 secara keseluruhan ( terutamanya standard&nbsp; 1- 7 (Hospital Wide Standard ) untuk Pensijilan Penuh 4 Tahun bagi MSQH Edisi 5.<\/strong><\/p>\r\n\r\n\r\n\r\n<h1 class=\"wp-block-heading\"><strong>SYARAT LULUS 4 TAHUN PENSIJILAN AKREDITASI MSQH EDISI 5<\/strong><\/h1>\r\n\r\n\r\n\r\n<p>Setiap service standard hendaklah mendapat rating 3 atau 4<br>Setiap core criteria di dalam standard hendaklah mendapat ratig 3 @ 4<br>Prestasi keseluruhan untuk setiap service standard juga bergantung kepada keadaan semasa tahap keselamatan pesakit dan anggotakerja dari segala perkara \/ aspek \/ elemen \/ anasir yang berada di sekeliling mereka.\u000b<br>KATEGORI BAGI PENSIJILAN AKREDITASI 4 TAHUN<br>PENSIJILAN AKREDITASI MSQH EDISI 5<br><strong>1. Kategori 1 ( Lulus Biasa )<\/strong><br>1.1 Semua service standard hanya mempunyai rating 3 atau 4 sahaja<br>1.2 Nilai peratus yang diperolehi dari pengiraan keseluruhan rating service standard dari 80 %.<br><strong>2. Kategori 2 ( Excellent Achievement Award )<\/strong><br>2.1 Semua service standard hanya mempunyai rating 3 &amp; 4 sahaja.<br>2.2 Nilai peratus yang diperolehi dari pengiraan keseluruhan rating services standard adalah 80 % dan ke atas.<br><strong>3.
