Application for Medical Facility Expenses<\/strong><\/td>\n\n

  1. Fill in Medical Form 1\/09 and attach a letter of clinical support from the doctor<\/li>\n
  2. Physicians need to confirm the disease and medications in Part IV<\/li>\n
  3. Bring along required documents to NF Pharmacy at Visitor Lobby<\/li>\n
  4. Medication supplies excluding supplements, vitamins and consignment items<\/li>\n<\/ol>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n
