  1. Staff of HCTM and UKM<\/li>\n
  2. Compulsory Retirement\/ EPF<\/li>\n
  3. Registered dependents<\/li>\n<\/ol>\n

    The required documents<\/strong><\/p>\n

    1. Valid original HCTM prescription + 1 copy<\/li>\n
    2. Completed 1\/09 Medical Form.<\/li>\n
    3. Patient identity card \/ pensioner card<\/li>\n<\/ol>\n

      \u00a0How to Apply for Medical Facility Expenses<\/strong><\/p>\n

        \n
      1. Fill up the Medical Form 1\/09<\/li>\n
      2. Doctor\/ Physician would need to fill-in to confirm the disease and medication in Part IV<\/li>\n
      3. Bring along your GL \/ Copy of UKM Retirement Card & original proof of Retirement from HCTM including HCTM prescription (plus a copy of it) to NF Pharmacy<\/li>\n
      4. Medication supply for a maximum of 3 months. Not including supplements, vitamins, consignment items etc.<\/li>\n
      5. Value of medication must be less than RM20,000 per prescription<\/li>\n
      6. If value of medication exceeds RM20,000, patients need to firstly obtain KKM\u2019s endorsement through the Governance Division, Jabatan Pendaftaran UKM<\/li>\n<\/ol>\n
