  1. Provide and ensure accurate, correct, relevant and up-to-date information at all times regarding personal information, medical needs, health history, treatment information, medications taken and used together with health-related matters intended for diagnostics, treatment, recovery / rehabilitation and purpose of the counseling/advice.
  1. Maintain, protect and usage of facilities, environment and service systems provided in good condition without abusing or damaging it.
  1. Know the services provided.
  1. Comply with hospital policies and regulations
  1. Adhere to treatment plans/rehabilitation/recovery procedures/advice/counseling.
  1. Obtain information on treatment costs and manage payments.
  1. Responsible for any/every information/treatment record that you own.
  1. Follow all recommended treatment plans by the specialist/staff responsible for patient care.
  1. Responsible for the effects of refusing treatment/advice or if not following the instructions/advice/notification by the staff responsible for patient care.
  1. Ensure that treatment bills/ charges imposed are paid within the prescribed period based on hospital rules and policies.
  1. Be considerate of the rights and comfort of other patients and hospital staff.
  1. Obtain proper information and if there were any inquiries, to ask the responsible party.
  1. Ensure that the conditions of treatment and surgical consent are understood.
  1. Ensure the presence of disabled patients/elderly to the hospital is accompanied by a caregiver who is able to manage the needs of patients such as can provide information and can ensure the continuity of care as recommended/planned.
  1. Responsible for ensuring the safety of personal belonging while being in HCTM premises.
  1. Knowing and complying with safety and emergency rules and actions whilst being in the premises.
  1. Respect the staff who are performing their duties and must avoid any form of harassment; be it physical, mental, sexual and other forms of harassment.
  1. Do not bring any dangerous equipment and illegal items into the premises.
  1. Do not bring NON-HALAL food/beverages to HCTM premises.
  1. Do not do any activity to solicit financial assistance from other patients/staff. All patient financial assistance applications must go through the processes and procedures set by HCTM.
  1. Comply with the ban on the use of cameras / mobile phones for the purpose of taking pictures / video / audio without written permission from the patient and the ban on taking pictures / video / audio in the ward / clinic or any area in the HCTM Complex for any purposes.
