GENERAL DOCUMENTS (MANDATORY TO BRING ALONG)

  1. Identification card / Passport (non-citizen) / Birth Certificate (children)
  2. Retiree card / Original JKM (OKU) Card (For OKU only) / letter of guarantee or approval of assistance
  3. Latest Employer/Panel Guarantee Letter (If applicable)

OTHER SUPPORTING DOCUMENTS NEEDED

  1. Government school students up to 17 years old – student confirmation letter
  2. IPTA students – letter of guarantee from the IPTA
  3. Blood donor – MOH blood donor card
  4. Organ donor – MOH organ donor card
  5. Infectious Disease Treatment Book – TB

DOCUMENTS INVOLVING REFUND

If it involves a refund, the following documents are required:-

  1. Original receipt
  2. Identity card (citizens) or passport (non-citizens)
  3. Confirmation of refund from clinic/ward/pharmacy/radiology/laboratory/emergency
  4. Relevant invoices or supporting documents

If the patient fails to submit the required documents, the patient will be charged.
