Document Requirements
  1. A valid original prescription from the relevant clinic as well as a photocopy
  2. Copy of Retirement Card
  3. Copy of Identity Card
  4. Original and copy of medical form 1/09
Application for Medical Facility Expenses
  1. Fill in Medical Form 1/09 and attach a letter of clinical support from the doctor
  2. Physicians need to confirm the disease and medications in Part IV
  3. Bring along required documents to NF Pharmacy at Visitor Lobby
  4. Medication supplies excluding supplements, vitamins and consignment items
