Eligibility UKM/HCTM staff and their dependants UKM/HCTM retirees and their dependants (husband/wife)
Document requirement
  1. A valid original prescription from the relevant clinic
  2. Employee card
  3. Guarantee Letter (GL) (for dependants)
  1. A valid original prescription from the relevant clinic
  2. UKM/HCTM Retiree Card
  3. JPA Retiree Card (for dependants)
How to apply for medical facility expenses
  1. Obtain a specialist stamp from the relevant clinic on the original prescription
  2. Bring along required documents to NF Pharmacy
  3. Medicine supply for 2 months, excluding supplements, vitamins and consignment items

  1. HCTM patients
  2. Medication cannot be supplied free of charge
  3. UKM/ HCTM staff/ retirees or dependants
  4. Retiree card/GL + Valid prescription (with physician’s signature)
  5. Other medication
  6. Pharmacy NF drug supply
  7. Hyaluronic Acid Inj: (> RM1,000/inj)
  8. Pharmacy NF Pharmacy send to BPP, MOH for approval
  9. NF Pharmacy supplies as approved
  10. Prescriptions worth> RM10,000 and Biologics
  11. NF Pharmacy send to the Director’s Office for approval
